Hypofibrinolysis in insulin resistance – (04-08-04)

February 17, 2013 by James Bogash

Hypofibrinolysis in insulin resistance: implication in cardiovascular diseases

The research linking poor insulin sensitivity is now so strong as to be unquestionable. One of the problems, however, is a good, solid lab test that can be used to diagnosis and monitor insulin resistance. Plasminogen activator inhibitor-1 (PAI-1) may turn out to be the marker we are looking for. Stay tuned…

TYPE 2 DIABETES MELLITUS AS A RISK FACTOR FOR STROKE – (02-27-06)

June 10, 2012 by James Bogash

Metabolic Syndrome Compared With Type 2 Diabetes Mellitus as a Risk Factor for Stroke

So metabolic syndrome was a greater predictor of stroke that diabetes, not because of the amount of risk it confers, but because metabolic syndrome is so darn prevalent. As high as 30%. 30%. Did I mention that the prevalence was as high as 30%? So why, I beg, is this condition so incredibly, blatently ignored?

There seems to be a never ending supply of bloodwork from patients coming into my office with strong family or personal histories of cardiovascular disease, and yet their bloodwork is limited to fasting glucose, standard lipids, and maybe an HbA1c. No fasting insulin, CRP, PAI-1, homocysteine. I’d pull my hair out in frustration, but I’ve none left…

BALANCE OF VISCERAL VS SUBCUTANEOUS FAT – (04-04-05)

May 8, 2012 by James Bogash

Differential Responses of Visceral and Subcutaneous Fat Depots to Nutrients

Wow.  What an insightful approach.  Basically, visceral (bad) and subcutaneous fat have a different response in the level of release of hormones after a glucose load.  The hormones derived from adipose tissue (resistin, leptin, adinopectin, PAI-1) generally contribute to insulin resistance and obesity.  More is released by visceral fat after feeding.

So the authors have planted a seed–might checking blood levels in non-fasting states give us more insight into the balance of visceral vs subcutaneous fat?  Interesting, huh?
