Diet Affects Lung Health

Many patients today are looking for quick fixes to their problems, whether that fix is a prescription drug or an herb or vitamin depends on the current attitude in healthcare. But man will never be able to even come close to mimicing nature. Because of this, diet also has and always will have incredible power in improving and maintaining health.

Breathing and eating are known to interact with each other, for better and for worse, and the food people choose to eat may lastingly affect respiratory function. At the World Congress on Lung Diseases in Florence, Italy, many scientific communications have attempted to unravel the complex relations between respiratory disorders and food. A recent large-scale survey conducted on 3000 men in Italy, Finland, and the Netherlands found that regular consumption of fruit and green vegetables improved respiratory capacity. In Italy, this benefit is believed to be due to the vitamin C contained in fresh fruit, whereas in Finland the effect would stem from the vitamin E found in abundance in vegetable oils. In the Netherlands, the scientists concluded that the protective effect was due to the beta-carotene contained in berries or yellow pulp fruit and in vegetables. A British team reported, after analyzing the lung capacity of 2500 Welshmen, that eating apples was a good way to ward off respiratory diseases. The expiratory volume of subjects who ate 5 apples a week was 140 mL greater on average than that of persons who ate none. These examples show clearly how food can affect breathing. But the reverse is also true, as was noted at the World Congress on Lung Diseases, and some disorders classified at first sight as respiratory diseases may affect the nutritional condition of patients.

Calcium Citrate Shown to Have Superior Bioavailability

Calcium carbonate has been recommended by many doctors who are not informed on the latest research, with many patients taking Tums and other calcium carbonate sources. Carbonate forms require stomach acid to be absorbed, and many patients are hypochlorhydric (not enough stomach acid) and become more so with age. In addition, many calcium sources are coming under fire for containing heavy metals such as aluminum and lead. Calcium citrate and MCHC are probably the most optimal sources of calcium. And, don’t forget that calcium is not all that is needed for bone health; we need numerous other minerals and vitamins, which are not typically found in most calcium supplements.

J Clin Pharmacol. 2000;40:1237-1244 The study, published in the November issue of the Journal of Clinical Pharmacology, used 3 measures to determine calcium bioavailability — serum calcium, urinary calcium, and serum parathyroid hormone (PTH). This randomized crossover study compared the single-dose bioavailability and effects on PTH of commercial calcium citrate 250 mg (Citracal, Mission Pharmacal) and calcium carbonate 500 mg (Os-Cal, SmithKline Beecham) supplements in postmenopausal women. Dr. Heller continued, “calcium citrate produced greater suppression of serum PTH by more than 50% over calcium carbonate. This provides physiological evidence that calcium citrate was better absorbed. We additionally found that calcium citrate may be particularly advantageous in those who absorb calcium poorly from calcium carbonate. Our data suggests that there is an important difference in bioavailability between calcium supplements in postmenopausal women. Careful long-term studies should be done to determine if there is an equal difference in protection against bone loss and fracture.”

The Hospital Water Supply as a Source of Nosocomial Infections

I’ve always said that hospitals are a dangerous place to be!! Chalk up yet another reason. I have to relay a recent experience. I had one of my oldest patient fall and break her hip. Hip replacement surgery followed and she was placed into a rehab/nursing facility. I gave her daughter Vitamin C and zinc (both are excellent for wound healing — vitamin C can be a limiting factor for formation of connective tissue; without it healing can slow down). The nursing staff went apoplexic and took them away and said they needed to ask the doctor. Of course, the doctor denied them and wrote a prescription for multivitamin (which are notoriously low quality). The ignorance of this situation is astounding and all too familiar.

Vitamins Use in a Male Physicians and Subsequent CVD

This article found no protective effect for multivitamins or vitamins C or E on cardiovascular disease. Before you run to the cabinet and throw away your Centrum multis (on second thought…if you are taking Centrum….) consider a few things. These articles rarely look into the type of multivitamin that the participant is taking. Multi quality varies incredibly, and my biased opinion would be that the researchers would have a hard time identifying characteristics of a high quality multivitamin. Just because a multi has 100% of the RDA does not mean it will be absorbed. Many nurses in the past have referred to Centrums as “bedpan bullets.” And ask any radiologist if they have identified intact vitamins on Xray.  My guess would be that a good, high quality multivitamin WILL lower risk of many diseases, CVD included.

Considering that many prenatal vitamins (preferably the non-pharmaceutical brands…) are high in folic acid, the results of this study are not surprising. Folic acid is essential for proper differentiation of many types of cells. Providing adequate levels of folic acid may help ensure that cells of the developing fetus are well differentiated (i.e. less likely to become tumors).

It’s always good to know what forms of vitamins and minerals are better absorbed, and it’s even better when the studies confirm what you’ve been telling patients for years, now. Magnesium oxide was no different from placebo. Let me put this simply. Magnesium–important. Magnesium–common deficiency. Magnesium oxide–bad. 80% of multivitamins–avoid.

Incidence of Recurrent Venous Thromboembolism After Treatment w/ Vitamin K Antagonists in Relation to Time Since First Event

This one is kind of an important one for me because I’ve had numerous patients in my office who have been on blood thinners for years for an isolated event many, many years ago. Basically, there doctors where afraid to take them off of the blood thinners for fear of recurrence. I remember a recent patient that had a clot in the femoral artery probably 10 years prior and had been on thinners since, with getting his blood checked every two weeks and the works. Fine and dandy, right? No future risk of stroke, right? This patient has Parkinson’s that was progressing. High dose vitamin E and ginko may protect against progression but these are no-nos with blood thinners (or so the “standard of care” would have us believe). This patient needed to have a serious heart-to-heart with his PCP and decide on his options. This study finds that after 9 months, risk from a one-time incident is negligent and blood thinners may no longer be needed.

Effect of vitamins and aspirin on markers of platelet activation – (06-23-03)

Effect of vitamins and aspirin on markers of platelet activation, oxidative stress and homocysteine in people at high risk of dementia.

It was nice to see therapeutic levels of the vitamins used in this study. Many times the researchers will use levels just barely above the RDA and when the findings do not show a benefit, the results are plastered all over the news. Here we see 1 mg of B12 (something like 15,000 times the RDA) and 2 mg of folic acid (5X). The antioxidant levels are not as high as I would like but they still showed lowering of markers of oxidative stress.
