Arterial Stiffness Increases With Deteriorating Glucose Tolerance Status.

More and more evidence is mounting for the contribution that insulin resistance plays in vascular dynamics, both directly and indirectly. Insulin resistance slows the breakdown of ADMA levels, thus blocking nitric oxide’s vasodilatory effects. Hyperinsulinemia lowers contractility of blood vessels. Insulin resistance increases homocysteine levels which then affects formation of plaque on the arterial walls. The list goes on, and one thing is clear–ANY patient with any degree of high blood pressure needs to address the issue of insulin resistance.

Prevention of type 2 diabetes

This is a nice review of a research study that showed that lifestyle changes can have a major impact in lowering risk of diabetes. The interesting note to this is that the authors used impaired glucose tolerance as a marker for risk of diabetes. The ironic thing about this is that impaired glucose tolerance is virtually unheard of in mainstream medicine, and yet lab values identifying insulin resistance can yield very clinically important information. And with this information lifestyle changes can be implemented to lower risk of diabetes.

Impaired glucose tolerance: GPs’ knowledge and perceptions

Anyone who’s even read this newsletter just once can probably grasp the medical fact that increased insulin resistance is probably the number one health concern in Western societies because of its impact on human health and chronic disease. So every physician should keep this entity #1 in their minds when evaluating any patient with CVD, hypertension, diabetes, obesity, menstrual disorders and a hoard of other conditions, right? WRONG…very wrong. The results of this survey of 34 PCPs in England gives us an indication that many have not cracked a journal in about 6 years. Or that pharmaceutical companies don’t sponsor seminars on insulin resistance. This is a sad, sad state of affairs for a condition that has strong potential to be managed with lifestyle interventions and affecting a major chunk of current chronic diseases in any physician’s practice.

As much as people don’t want to hear it, I consider Alzheimer’s and many other forms of dementia as consequences of a Westernized diet. Here we see that poor glycemic control (dysglycemia, Syndrome X…) leads to poor mental functioning performance. Anyone worried about memory and brain performance needs to address glucose control.

Glucose Tolerance during Moderate Alcohol Intake

This article suggests that moderate alcohol intake can increase insulin sensitivity. The key word here is moderation. I would recommend the use of red wine to the tune of 1-2 glasses several times per week. Red wine has the additional benefits from the reservatrol; a bioflavonoid that has been shown to lower risk of CVD as well as certain types of cancer.

INSULIN RESISTANCE – A MAJOR CONTRIBUTOR TO PLAQUING – (08-13-07)

Nontraditional atherosclerotic risk factors and extent of coronary atherosclerosis in patients with combined impaired fasting glucose and impaired glucose tolerance.

There are many clinical trials attempting to find that one drug that, when given at high dosages, can reverse or stop plaquing of the arteries. Of course, all this money spent is in the face of lifestyle changes and natural compounds that already to what the drug companies are trying to mimic.

This study found a relationship between the amount of plaquing in the coronary arteries and several “non-traditional” risk factors. So often physicians view coronary heart disease risk with blinders on — check cholesterol, blood pressure, family history, etc… But these risk factors only show a small part of what’s going on as far as risk of CHD in the patient.

This study found that insulin resistance was a major contributor to plaquing. No big surprise, of course, given how pro-inflammatory insulin resistance is.

RED WINE INTAKE OF MODERATE AMOUNT SHOWS A POSITIVE HEALTH BENEFIT – (03-17-08)

Life style habits such as alcohol consumption and physical activity in relation to serum apoB / apoA-I ratio amongst 64-year-old women with varying degrees of glucose tolerance

Alcohol is always a hot topic when it comes to health. In general, almost all the studies looking at red wine intake of moderate amount (up to 1/2 bottle / day) show a positive benefit. This study is no different, although they did not look at type of alcohol consumed. Of note in this study was that alcohol intake above one glass per day was needed to show a positive effect.

Also, this article had an interesting finding; exercise “leading to sweating” had a positive effect. I don’t think I’ve ever seen the term “leading to sweating” used, but it gives us a real good idea of what level was needed. Walking in many cases is probably not enough. So, try jogging on the treadmill holding a glass of red wine.
