I remember some years back getting into an email “argument” with a cardiologist who thought that my estimates of the very high prevalence of cardiovascular patients that were insulin resistant was exceedingly high. He thought maybe 2%. Talk about way off. I just hope he’s not actually practicing cardiology…

This article found sugar handling problems in up to 57% of the patients w/ CVD. Now, math was never my strong point, but isn’t 57 > 2??

Wow. Sometimes we really hit articles that remind us how far we can fall behind in the research. Most physicians are still blissfully making clinical decisions based entirely on total cholesterol. Some even take HDL into account. Smaller still are the physicians recognizing the risk that triglycerides/HDL ratio play. So, while most physicians are decades behind using tri/HDL ratios, research is finding that apoB/apo A-1 ratios are much, much stronger at predicting CVD risk. DIETARY CHOLESTEROL AND EGGS LEAD TO CORONARY HEART DISEASES – (04-10-06)

Dietary cholesterol, eggs and coronary heart disease risk in perspective

The concept that dietary cholesterol leads to a direct elevation in blood cholesterol and subsequent risk of CVD has been promoted for some time now, despite evidence to the contrary.

This article reviews egg’s “bad rap” and the lack of validity to the idea that eggs raise contribute to CVD. I recommend eggs as a good protein source for breakfast, and steer patients towards the “veg-a-fed” type. These animals are fed a diet high in omega-3 fatty acids, leading to a higher content of omega-3 fatty acids in the egg.

Prediction of Lifetime Risk for Cardiovascular Disease by Risk Factor Burden at 50 Years of Age

Wow. The difference in risk between those without CVD by age 50 compared with those diagnosed w/ CVD by age 50 is quite dramatic. The problem is that is remains difficult to get young people (i.e. under 50) to understand the dangers that their lifestyle is producing. It also makes you wonder what’s going to happen in the next few decades as our unhealthy moms have unhealthy children and these kids get raised in unhealthy households. It seems that each generation is noting the onset of chronic disease earlier and earlier.

METABOLIC SYNDROME EFFECTS THE HEART ATTACK CONSEQUENCES – (03-27-06)

Benefits of Niacin in Patients With Versus Without the Metabolic Syndrome and Healed Myocardial Infarction (from the Coronary Drug Project)

So you’ve had a heart attack. The cardiologist must have lots of wonderful drugs they can chose from to keep you from having another heart attack, right? How about those highly prescribed and wonderfully advertised statin drugs? They ought to work wonders.

Unfortunately for the drug companies, in this moderately large study, only niacin was able to prevent second heart attacks. The catch? It was only in patients with metabolic syndrome. What a kicker, right? Few doctors recognize the importance of identification of metabolic syndrome, and yet this delineation could mean the difference between life and death if you’ve had a heart attack.

And, considering that some studies put the incidence of metabolic syndrome as high as 88% in CVD patients, you odds are very high…

LOW GLYCEMIC INDEX DIET RISKS POSTMENOPAUSAL WOMEN – (03-27-06)

Effect of a low glycemic index diet with soy protein and phytosterols on CVD risk factors in postmenopausal women

I have frequently voiced my unhappiness with most of the national anti-disease organizations. They take a very soft approach and seems like industry probably has a large voice in their recommendations. We have seen study after study finding stronger effects and better tolerability of a Mediterranean diet and low glycemic index diet vs the American Heart Association step 1 diet.

I guess my question is, with all the research coming out over the years, why haven’t the AHA changed their recommendations?

FACTORS THAT COULD LOWER RISK OF HEART DISEASE AND STROKE – (02-13-06)

Riboflavin Lowers Homocysteine in individuals Homozygous for the MTHFR 677C to T Polymorphism.

This is a perfect example of a problem with medicine and research today. Awhile back there was some debate over whether checking homocysteine added any predictive value to determining CVD risk and whether the addition of B12 and folic acid could lower risk of heart disease and stroke. Many said the effect was not strong enough to warrant the “hassle.” Now we see that riboflavin also plays a large role in this pathway, and in patients with the MTHFR genotype that had low riboflavin levels to start with, the reduction in homocysteine levels was a high as 40%.

So, any studies that did not find a strong effect of B12 and folic acid for lowering CVD risk now needs to go back to their data and see if the addition of riboflavin would’ve affected the results.
