Insulin Resistance Increases the Risk of Spontaneous Abortion after Assisted Reproduction Technology Treatment

“Patients with IR should be advised to improve their insulin sensitivity through lifestyle change or medical intervention before infertility treatment to reduce their risk of spontaneous abortion.” HAH!! Like that would ever happen!! Why change anything when IVF is available on every corner?

The bottom line is that we cannot and should not ever try to cheat Mother Nature. There will always be a price to be paid. The rapid increase in IVF pregnancies will come with a price to society–as unhealthy mothers give birth to predisposed babies who are raised in the same unhealthy manner we will start seeing chronic diseases begin earlier and earlier in age

Clomiphene, Metformin, or Both for Infertility in the Polycystic Ovary Syndrome

Jese o’ Pete–How about NEITHER!! PCOS is a lifestyle condition that needs to be managed from a lifestyle standpoint. You force ovulation on a body that is saying it doesn’t want to get pregnant and you grow a fetus in an unhealthy system and then mom raises that child in the same poor lifestyle she had. This is a deadly combination as it pertains to chronic disease.

Although we are seeing the onset of chronic disease in younger and younger patients each generation, I firmly believe that we have only seen the tip of the iceberg as it relates to chronic disease.

SOY INTAKE REDUCES BREAST, ENDOMETRIAL AND PROSTATE CANCER RISK – (07-30-07)

Epidemiologic evidence suggests that dietary phytoestrogen intake is associated with reduced risk of breast, endometrial, and prostate cancers

It amazes me how many anti-soy sentiments there are out there. Thryoid cancer, breast cancer, growing breasts on little boys, thyroid problems, infertility…the list is quite long. However, I have looked at many of the references the “anti-soy” people use. They just don’t hold up. I remember letter to the editor I wrote in response to an article damning soy. The journal let the author of this article rebut my letter. I checked her references and, out of 7 or so, 5 were in SUPPORT of soy!! She never checked her references!!

The vast majority of research articles on soy and phytoestrogens are positive, this one included. And I find it biased that there is concern over the phytates in soy blocking mineral absorption, but tea containing tannins are ok. Soy containing phytoestrogens are of concern, but eat all the lignans in flax you can grind up. You can’t have it both ways. That being said, I am in support of soy as being a part of a healthy lifestyle, but that does NOT mean processed soy this and processed soy that. Tofu, miso, soy nuts, soy milk, edemame–these are closer to Mother Nature’s intent. Textured vegetable protein and veggie burgers are less in Mother Nature’s plan.

INFERTILITY TREATMENT AND CONGENITAL MALFORMATIONS – (03-05-07)

Infertility, infertility treatment, and congenital malformations

This is really a very bad omen. I’ve said it before and I will say it again. We are messing with Darwin and there will be a serious price to be paid for our intervention. Forget Maslow’s Heirarchy of Needs–the ONLY reason a species exists is to propagate its own. So, if a couple is infertle, there is a definite reason why Mother Nature does not want this couple to reproduce.

Many times there is a physiological problem that needs to be fixed and before the couple could become pregnant. If we don’t fix that problem and we force a pregnancy onto this couple, we have dramatically shifted the balance of Nature. There is a point at which medicine needs to decide that the best intervention is NO intervention.

POSITIVE HEALTH BENEFITS OF SOY DECREASES RISK OF PROSTATE CANCER – (02-04-08)

Dietary Isoflavones May Protect against Prostate Cancer in Japanese Men

There are no shortage of websites out there touting that soy is killing us all, causing infertility, breast cancer, thyroid cancer and mineral deficiencies. The research just doesn’t match all this fear. While allergy is a concern, I feel it’s more from overdosing infants with soy-based formulas then anything else.

Here we have yet another study finding positive health benefits of soy, with men with the highest levels of isoflavones in the blood having a whopping 58% decreased risk of prostate cancer in Japanese men when compared to men with the lowest. Now, there are other things to consider here. Not only is the intake of soy products important, but we have to have the right type of bacterial flora in the gut to convert the isoflavones in soy to their more potent counterparts–the aglycone form. So antibiotic use could drastically alter the protective effect of soy on risk of prostate cancer. So…next time your PCP gives you that antibiotic prescription for a viral infection, ask how this is going to alter your risk of prostate cancer by destruction of normal flora.

INSLIN SENSITIVITY AFTER METFORMIN SUSPENSION – (02-11-08)

Insulin Sensitivity after Metformin Suspension in Normal Weight Women with PCOS.

I have long asserted that we are handling infertility in a very poor manner. In most cases, infertility is related to poor lifestyle, and not addressing these lifestyle factors opens up an entire Pandora’s Box of potential problems, both with pregnancy, pregnancy outcomes and the long term health of the child.So, the ONLY real answer is lifestyle changes. But alas, many women with PCOS (the main cause of infertility here in the US) are just given Metformin, a drug that increases insulin sensitivity in the liver.

 This study evaluated what happened when those patients were taken OFF of this drug. Interestingly, the women with PCOS were WORSE OFF after the treatment then before!! It just reinforces the idea that drugs are not the answer for this condition, and any other approach that doesn’t address lifestyle is likely to make the condition worse. Also, consider that Metformin only works in the liver. A few articles back, we saw that insulin resistance in the muscle is a major factor in the development of MetS, and yet Metformin does nothing to address this aspect.

POSTNATAL SPECIFIC APPETITES AND FEEDING BEHAVIOR – (05-25-09)

This phenomena comes up again and again and again and again in the medical literature. There is no longer any doubt that in-utero exposure to a variety of chemicals, situations and physiological states have an influence on the newborn both immediately and decades in the future. This is NOT being communicated to women of childbearing age nor to pregnant women at the level of concern that it needs to be.

Sure, pregnant women should not be smoking, but exposure to BPA and other chemicals, rapid weight gain, stress, gestational diabetes, high refined carbs, high saturated fats, low omega 3 fatty acids and low phytonutrient intake ALL need to be on the plan for every women even considering becoming pregnant. And how does this all impact the woman who’s body is not ready for pregnancy (i.e. infertility), but is forced into it anyway (infertility treatment)?

TEENAGE DAUGHTER HAVING MENSTRUAL PROBLEMS?

Let’s get something clear right now. The presence of polycystic ovarian syndrome (PCOS) in a teenager is very serious business. It is NOT a situation to be managed with medications or the birth control pill (which will, ultimately, make the condition worse).

The long term ramifications are immense. Given that this IS prediabetes, all those risks are present. Heart disease, diabetes, liver problems (NASH). Worse, if this young adult thinks she may have a desire for children 10 or 20 years in the future, it needs to be managed NOW. PCOS is the most common cause of infertility in women.

While a pharmaceutically oriented physician would give lifestyle changes for PCOS nary a second glance, the research is heavily in support of the use of lifestyle changes to manage the problems associated with PCOS. As a matter of fact, not making lifestyle changes and taking the drug route has been shown to worsen PCOS.

This current study found changes in all aspects of PCOS with 1 year of lifestyle changes, exercise and counseling. Changes included:

  • Lowered body mass
  • Blood vessel health improved (intima media thickness)
  • Testosterone levels were lowered and SHBG levels went up
  • Menstrual cycles improved (amenorrhea and oligomenorrhea by 42% and 19%, respectively)
  • Lowered insulin levels

These are all very powerful improvements and show that lifestyle changes are superior to medications for managing this condition. So begin an anti-diabetic lifestyle now. Otherwise you may never get a chance to bounce that grandchild on your knee.
