Insulin Resistance in Muscle, Liver May Be Triggered by Fat Cell Defect

This is a rather complex article looking at the commonly held belief that fat cells do not have an influence on insulin resistance; rather, they are believed to suffer the effects of insulin resistance occurring in muscle and liver. This may not be so. This study used mice to initiate insulin resistance only in fat cells. Soon, changes in insulin sensitivity were also seen in the liver and muscle cells. In English, this means that the fat cells send out some (currently called “resistin”) hormone that leads to insulin resistance in other cells. And remember that insulin resistance is a major contributor to diabetes. So much for calories in, calories out being the only thing you need to worry about for weight loss!!

Nature. 2001;409(6820) Fat cells that can’t take up blood sugar normally appear to trigger the same problem in muscle and a related problem in liver, say researchers from Beth Israel Deaconess Medical Center in Boston, Massachusetts. Using a sophisticated genetic approach, the researchers wiped out the key protein in mouse fat cells that insulin tells to move glucose from the blood into the cells. Soon, muscle also ignored insulin’s orders to take up glucose, and liver disregarded insulin’s instructions to shut down the organ’s glucose production. “This paper clearly shows that fat also is important for whole-body insulin action. We think fat releases a molecule that circulates to muscle and liver and impairs insulin signaling in those cells, ” says senior author Barbara Kahn, MD, chief of the division of endocrinology and metabolism at Beth Israel Deaconess Medical Center. “For years, people have thought — and data have shown — that glucose uptake in muscle is very important for normal insulin action and to prevent diabetes. researchers thought the decreased glucose uptake into fat of obese and diabetic people was not important in causing insulin resistance,” says Kahn, also a professor of medicine at Harvard Medical School. The new study reinforces a new appreciation of fat as an endocrine organ that influences other tissues. In January, for example, other researchers identified a hormone produced by fat cells, dubbed resistin, that prompts other tissues to resist insulin. Resistin is the latest in the list of active factors known to be secreted by fat that can affect insulin action. The list includes tumor necrosis factor-alpha, fatty acids and leptin, which works through the brain to regulate feeding and metabolism. Kahn’s research group excludes all of these as likely culprits in the new paper.

This is a nice review of a research study that showed that lifestyle changes can have a major impact in lowering risk of diabetes. The interesting note to this is that the authors used impaired glucose tolerance as a marker for risk of diabetes. The ironic thing about this is that impaired glucose tolerance is virtually unheard of in mainstream medicine, and yet lab values identifying insulin resistance can yield very clinically important information. And with this information lifestyle changes can be implemented to lower risk of diabetes.

Effects of Oral Steroidal Contraception and Insulin Resistance

Consider a treatment that is considered benign by many women and doctors such as the birth control pill. This has been taken to the extent that some have even suggested that women need not have periods anymore; they can be eradicated through the use of BCPs. Remember that any time you play with the major hormones in a woman’s (or man’s) body, you are playing with a dance that Mother Nature created and we do not fully understand. The fact that BCP contributes to insulin resistance has been known for some time now, but it is nice to see the topic pop up now again in the medical literature. Many women are given BCP for “control” of ovarian cysts; polycystic ovary disease has a strong link to increased insulin resistance. Does this sound like it should be the therapy of choice?

Adiponectin reverses insulin resistance

Here is another example of a newly discovered hormone that plays a role in insulin/weight management. Keep an eye out for this one; this is a mouse study but the impact on human health is huge. The fat-derived hormone adiponectin reverses insulin resistance associated with both lipoatrophy and obesity.

The metabolic syndrome, or Syndrome X, is a state of insulin resistance that leads to a wide variety of other signs and symptoms in the patient. It is well known that insulin levels play a large role in heart disease, and this study adds further weight to this idea. Any patient with or at risk for heart disease needs to adopt a lifestyle that pays attention to lowering insulin resistance and improving sensitivity.

Remember from past updates that hyperinsulinema affects the cells lining the arteries and because of this may be the answer behind supposedly “essential” hypertension. This article supports that hypothesis. On the scarier side, how many of our young people now have increased insulin resistance because of the nutrient poor, highly processed dietary patterns that are all too common?

55th Annual Fall Conference of the AHA Council for High BP Research In 15-year-old males, insulin resistance correlates with systolic blood pressure. Dr. Alan R. Sinaiko of the University of Minnesota, Minneapolis and colleagues from Wake Forest University in Winston-Salem, North Carolina used euglycemic insulin clamp studies to measure insulin resistance in 159 boys and 128 girls enrolled in Minneapolis Public Schools. Measurements were collected at age 13 and again at 15. Half of the volunteers came from the top 25% of the school population based on socioeconomic status. Fifty-three students were black, Dr. Sinaiko said. On average the boys had a higher body mass index (BMI) than girls at both age 13 and age 15. Results of the studies done at age 13 did not indicate an association between insulin resistance and systolic blood pressure, but “in 15-year-old males there was a significant association.” For both males and females, higher insulin resistance was also associated with higher triglycerides and lower HDL. There was no correlation with diastolic blood pressure. Moreover, as in adults, “insulin resistance and systolic blood pressure was influenced by total body fatness,” Dr. Sinaiko said.

Regular readers of the Updates will hopefully now realize that the root of much of the evil in civilized nations is refined carbs. Pick a disease and insulin resistance is linked somehow. We need so focus much of our efforts into educating the public about the difference between whole grains and refined carbs. This article looks at NORMAL levels that are on the high end and finds links with several serious complications of pregnancy.
