For participants in the Firstline Therapy program, diet history services are included in the program. For non-participants, there is a $50 fee for a one week comprehensive review. Depending on the level of detail and how often you would like to send your log in, we offer 3 types of logs: A detailed daily log, a short daily log or a weekly log. Please select one of the following logs:
